Age Limit: Candidates
age should be 33 years for DM, MCh, 35 years for PDCC as on 31-12-2015. Age
relaxation is applicable 5 years for SC/ ST/ OBC/ widowed/ divorced/
legally-separated women who have not re-married/ disabled/ handicapped
candidates of Uttar Pradesh as per Government rules.
Educational Qualification: Candidates
should possess MD/ DNB/ MS/ DM/ MCh (Medicine/ Pediatrics/ Obst. & Gyne/
Anaesthesiology or Internal Medicine or Respiratory/ Pulmonary Medicine or
Emergency Medicine/ Surgery/ Gastroenterology/ Neurology/ CVTS)/ M.Sc in
Medical Physics.
Selection Process: Candidates
will be selected based on Written test & Departmental test.
Application Fee: Candidates
have to pay Rs. 1000/- + Bank Transaction charges by online through Net
banking, Debit/ Credit card or through computer generated Challan form in any
branch of State Bank of India.

& send hard copy of online application along with all relevant documents
through speed post/ registered post only to Executive Registrar, SGPGIMS,
Raibareli Road, Lucknow-226014 on or before 25-11-2015. Superscribe the
envelope with Advertisement Number & “Application for entrance exam to SR
(HS)/ DM/ MCh/ PDCC/ PDF (————- Name of Specialty ———)”.

2. Click on “Announcements” —> “Appointment of Senior Resident (Hospital Services)/ Medical Physics Residents/ Senior Demonstrators in for the session January 2016″.
3. Read the information broucher & Click on “Online Form Submission” —> “Register & Pay Online” —> “Proceed”.
4. Fill all the details carefully & pay the fee & Submit the form.
5. A transaction alert will be sent to your registered mobile number.
6. After one clear working day, again log on to the website & Click on “Online form submission” then select “Complete form”.
7. Fill the transaction number and your date of birth
8. The main form will open, Fill all the details & Click on “Submit once and please wait” and upload relevant documents.
9. Now take printout of online application for future use.
